After completing the residency information, you will be asked to complete a Registration form, a Request for Records and a Home Language Questionnaire (find forms on forms page). We request that you bring the following paperwork with you:
- Child’s Birth Certificate

Any incomplete paperwork will need to be obtained as soon as possible.
General kindergarten registration for the Greater Johnstown School District will take place during April. Registration packets will be mailed to families with students who have already been identified for next year’s classes, either through pre-kindergarten or from local nursery school enrollments. If your child will not be identified through these means, please contact the District Office for a registration packet. Children must be five years old on or before December 1 to be eligible for kindergarten registration.
For open kindergarten registration during April, it is requested that parents/guardians contact the District Office to schedule a convenient time to complete the process. The following items must be brought to kindergarten registration.
- Child’s birth certificate
- Immunization record. Immunizations must be completed before your child enters school.
- School Entrance Health History (available in packet)
- Student Information Form (available in packet)
All parents will need to complete the residency form (found on the forms page) and bring it to the District Office in the high school building, together with proof of residency and other forms in the registration packet.
The kindergarten registration process will take approximately 15 minutes and children do not need to attend. Appointments will be made for screening in May.
Anyone who cannot attend kindergarten registration during April should contact the registration office prior to that date at (518) 736-1070.
All kindergarten classes are held at Pleasant Avenue School.
