GJSD students 12 years old and above

  • 1st Dose Thursday, May 20th – 8:30 a.m. – 2:30 p.m. at Johnstown Performing Arts Center (JPAC at the Junior-Senior High School)
  • 2nd Dose -Thursday, June 10th – 8:30 a.m. – 2:30 p.m. at Johnstown Performing Arts Center (JPAC at the Junior-Senior High School)

Participants will need to bring their driver’s license or another form of ID along with an insurance card. Without insurance, NYS will still cover the cost of the vaccine.

  2. Complete the NYS Department of Health Form.
    Participants will need to complete this from for NYS prior to the day of the clinic: https://forms.ny.gov/s3/vaccine
  3. Complete the Adirondack Health and Wellness Consent Form
    Please click this link, complete the form, and bring a signed copy with you the day of the clinic.  Ages 12 – 17 will need parental consent.
