GJSD hosts Pfizer vaccination clinic 9-11 a.m. on Friday, January 14

The clinic will be open to GJSD staff, students, and families of students, ages five and up. Only the Pfizer vaccination will be available that day and participants have the option of selecting: Dose 1, Dose 2, Booster or Pediatric 5-11.

Required forms for anyone under the age of 18: 

  • Adirondack Health and Wellness requires participants who are 5-17 years old to have parental consent in order to receive the vaccine. The form can be found here.
  • NY state also requires parental consent for anyone under the age of 18 (1st dose only; students may print the page and bring the completed form with them or may show a photo of the completed form):  https://forms.ny.gov/s3/vaccine
