COVID-19 Pfizer vaccination clinic @ Jr.-Sr. HS from 8:30-11:45 a.m. on Tuesday, July 13 (pre-registration required)
GJSD will hold a voluntary COVID-19 Pfizer vaccination clinic in partnership with Adirondack Health and Wellness for Johnstown students ages 12+ from 8:30 – 11:45 a.m. on Tuesday, July 13 at Johnstown Junior-Senior High School.
PRE-REGISTRATION IS REQUIRED (see below).
First doses are scheduled for Tuesday, July 13. Second doses will be administered at a clinic on Tuesday, August 3 at the same scheduled time as the student’s first dose appointments.
Required documents include a Department of Health questionnaire and a parental consent form for anyone under the age of 19. Pre-registration steps include:
- Make an appointment by clicking here.
- Complete the NYS Department of Health Form prior to the day of the clinic, by clicking here.
- Download the Adirondack Health and Wellness Consent Form by clicking here. Bring a signed copy with you the day of the clinic. Students under the age of 19 will need parental consent.
We may also be able to offer parents and caregivers accompanying their student to the clinic the opportunity to receive a vaccination appointment. Students, however, will have first priority.
Adirondack Health and Wellness will be accepting insurance information from individual families for the administration of the Pfizer vaccine. In the event a family is uninsured, the cost will be covered by NYS.
This is a first come, first served clinic. Additional clinics may be scheduled in the future, depending on demand.
