COVID-19 Pfizer vaccination clinic for students 12+ on Tuesday, Oct. 12 (pre-registration required)

a graphic shows a medical vaccination needle, with the text "COVID-19 Vaccine Clinic, Tuesday, October 12, Johnstown Jr.-Sr. High School"GJSD will hold a voluntary COVID-19 Pfizer vaccination clinic in partnership with Adirondack Health and Wellness for Johnstown students ages 12+ from 1:00-2:45 p.m. on Tuesday, Oct. 12 at Johnstown Junior-Senior High School. Students under the age of 19 will need parental consent. The second dose would be administered on Tuesday, Nov. 2 at the same scheduled time as the student’s first dose appointments.

Required Documents:

  1. Complete the NYS Department of Health Form prior to the day of the clinic, by clicking here.
  2. Download the Adirondack Health and Wellness Consent Form by clicking here.  Bring a signed copy with you the day of the clinic.

We may also be able to offer parents and caregivers accompanying their student to the clinic the opportunity to receive a vaccination appointment. Students, however, will have first priority.  

Adirondack Health and Wellness will be accepting insurance information from individual families for the administration of the Pfizer vaccine. In the event a family is uninsured, the cost will be covered by NYS.

This is a first come, first served clinic. Additional clinics may be scheduled in the future, depending on demand.
