New Student
Registration
In order to
register a new student, residency in the Greater Johnstown
School District must be verified at the District Office
located in the High School building. The
residency verification form
and all necessary registration forms are available on this web site, or the forms can be
obtained and completed at the Registration Office. For

Proof of residency must be provided using one of the
following:
After completing
the residency information, you will be asked to complete a
Registration form, a Request for Records and a Home
Language Questionnaire. We request that you bring the
following paperwork with you:

Child’s
Birth Certificate

Child’s
Immunization Record
-
Custody
Paperwork (if applicable)
-
Child’s IEP
(if applicable)
-
Name,
address and phone number for previous school
district
Any incomplete
paperwork will need to be obtained as soon as possible.
Kindergarten
Registration
General kindergarten registration for the Greater
Johnstown School District will take place during March and
April. Registration packets will be mailed
to families with students who have already been
identified for next year’s classes, either through
pre-kindergarten or from local nursery school
enrollments. If your child will not be identified
through these means, please contact the elementary
school closest to your home for a registration packet.
Children must be five years old on or before December 1 to be eligible for kindergarten registration.
For open registration during the end of March and/or
beginning of April, it is requested
that parents/guardians contact the school office to
schedule a convenient time to complete the process. The
following items must be brought to registration.

School Entrance Health History (available in packet)

Student Information Form (available in packet)
Unless you have a child currently enrolled in the
Johnstown School District, you will need to complete the
residency form and bring it to Alice Sise, Business
Manager, at the Johnstown School District Office, 1 Sir
Bills Circle, with proof of your residency.
Immunizations must be completed before your child enters
school.
The registration process will take approximately 15
minutes and children do not need to attend. Appointments
will be made for screening for the end of April or
beginning of May.
School of registration does not necessarily indicate
school of attendance. Final determination of school
of attendance will be made during the month of August,
after which parents will receive a letter from the
kindergarten teacher.
